FOCUSING ON THE WHOLE PATIENT
The Appalachian Team consists of dedicated medical professionals who focus on the physical, emotional, psychological, and spiritual needs of our patients. You can count on the Appalachian team for professional, family-oriented care and support.
All medical care is overseen by an Appalachian Hospice Medical Director, an Associate Medical Director or the patient’s attending physician. Our nurses provide pain and symptom management and monitor the patient’s condition on a routine and as-needed basis. Certified Nursing Assistants help with patient’s personal care needs.
A TEAM DEDICATED TO PERSONALIZED CARE
In addition to our dedicated professionals, Appalachian Hospice also works with a variety of other caring individuals to provide patient and family support including social workers, chaplains, and volunteers.
Social workers help with emotional and psychological concerns, assist with arranging financial and other community resources and provide counseling to patients and families. Chaplains provide non-denominational spiritual care and counseling while volunteers provide companionship and a variety of other assistance to help patients and their families.
Additionally, Appalachian Hospice provides:
A transitional care program to ensure the safe and positive transition of you or your loved one from hospital care to home care
Medical equipment, medical supplies, and medications related to hospice diagnosis
Respite Care
Bereavement counseling and support
Annual memorial services
Nutrition evaluations
INDIVIDUALIZED CARE PLANS
Each patient receives a complete and personalized assessment by a registered nurse, a licensed social worker, spiritual care counselor. All patient assessments are reviewed and approved by an Appalachian Hospice Medical Director or Associate Medical Director. These initial assessments, along with your input will guide all the Appalachian team members in completing a care plan tailored to the patient’s specific medical, social, comfort needs and desires. Your individualized care plan is monitored, evaluated, and updated routinely.
WHERE CAN SERVICES BE PROVIDED?
Hospice care can be provided in the home, in a long-term care facility, in an assisted or personal care facility or in a hospice in-patient facility.
